Physical Therapist

Position Overview
We’re seeking a licensed Physical Therapist to provide skilled therapy services across our centers. You’ll evaluate patients, develop personalized treatment plans, and collaborate with caregivers and facility staff to ensure optimal outcomes.

Key Responsibilities

• Deliver medically necessary physical therapy services
• Create and implement individualized treatment plans
• Supervise therapy assistants and aides
• Maintain accurate, timely documentation
• Communicate effectively with patients, families, and clinical teams
• Participate in facility meetings and continuing education
• Ensure compliance with Medicare, OSHA, and state regulations

Qualifications

• Graduation from a CAPTE-accredited Physical Therapy program 
• Active state licensure for California
• Strong leadership, communication, and organizational skills
• Ability to lift up to 50 lbs and perform physical tasks throughout the day
• Current CPR certification
