Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Illinois Physical Therapist license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience working in skilled nursing facility settings preferred.
- Experience managing caseloads in post-acute or long-term care environments preferred.
- Willingness to split assignment and travel between multiple local facilities (Tuscola, Arcola, Bement) as required by the client.
- Ability to work a full-time schedule and meet facility documentation and productivity expectations.

- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Shift Details: Days 08:30-16:30, 5x8 schedule.
- Assignment Structure: Split assignment between multiple local facilities within the Tuscola area; travel between sites required.
- Contract Weeks: 13-week assignment.
