This facility is seeking a Dentist for locum tenens support as they look to fill a current need.

Details and requirements for this opportunity:

· Schedule: Monday -Thursday, 8a-5p

· Setting: Clinic

· Procedures/Cases: General and Restorative Dentistry. Provider working out of 2-3 chairs with 2 Dental Assistants and 1-2 Hygienists. 98% of patients are adults. Procedures needed: Fillings, Orthodontics - Invisalign Certified (not required), Crowns, Bridges, Restore Implants, Cosmetic Dentistry

· Credentialing Timeframe: 1-2 days

· Requirements: State license, Drug Enforcement Administration license, Cardiopulmonary Resuscitation License, Basic Life Support License, National Provider Identifier Number, Certificate of Insurance, Client application. Negative Tuberculosis test within 1 year State license. All doctors must complete Heartland credentialing within 5 days of receiving link/emails from client through their Verifiable online portal - no exceptions.
