The Chalmers P Wylie Veterans Outpatient Clinic is recruiting full-time Social Workers. The incumbent, professional licensed clinical social worker, will function as a case manager for the VAACC's Housing and Urban Development/VA Supported Housing (HUD/VASH) program. The program is a specialized treatment program area, with collaborative effort with the local Public Housing Authority to provide homeless veterans with Section 8 Housing Choice Vouchers and long-term community case management.

- The incumbent serves as a case manager for the HUD-VASH Program. The incumbent is responsible for data collection, administrative record keeping and monitoring patient or clinical indicators, thresholds and performance measurements. Additionally, the incumbent maintains working relationships and agreements with other organizations and directly coordinates with the designated PHA regarding housing voucher application and award process for each veteran. The incumbent participates in the review of agreements between landlords, PHAs and the VA to establish appropriate referral sites for placement.
- The incumbent regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters and on the streets, as well as those who are referred by VA residential programs and facilities. The incumbent maintains current and establishes new relationships with other community-based service providers for the homeless, and when appropriate, represents VA at community service provider meetings.
- Incumbent has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illness/injuries, common medications and their effects and medical terminology. Has a general knowledge of veterans' benefits and services related to special programs, service connected compensation and non-service-connected pension benefits. Assists veterans with linkages to organizations that assist with the VA benefits application process.
- Incumbent assists the Coordinator, Community Outreach Division with ensuring the HUD VASH program meets CARF, SOARS and Joint Commission requirements.
- The incumbent will also be required to perform most (75%) of their job duties in the community and will be required to utilize a GSA vehicle while performing such duties.
- Able to meet the needs of customers while supporting VA missions. Consistently communicates and treats customers in a courteous, tactful and respectful manner. Incumbent is able to provide the customer with consistent information according to established policies and procedures. Handles conflicts and problems in dealing with the consumer constructively and appropriately. Has working knowledge and skill in the use of word-processing, data management and other computer systems.
Work Schedule: Monday through Friday, 8:00am-4:30pm.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA.
