Job Description


PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care.

Position Summary The PACE Clinical Coordinator serves as the primary point of contact for participants and caregivers within the clinic setting. This role provides clinical support and administrative coordination to ensure efficient daily operations. Responsibilities include appointment scheduling, participant intake and rooming, obtaining vital signs, maintaining exam room readiness, supporting clinical documentation, and assisting the interdisciplinary care team. When properly certified, the Clinical Coordinator may also perform basic phlebotomy procedures. The position supports high-quality participant care while maintaining compliance with regulatory and organizational standards. Essential Functions
  • Serve as the first point of contact for clinic participants and caregivers.
  • Schedule, monitor, and adjust clinic appointments to meet participant and operational needs.
  • Check participants in and out and maintain accurate scheduling records.
  • Room participants and prepare them for evaluation by clinical staff.
  • Obtain and document vital signs accurately.
  • Assist with clinical documentation and ensure medical records are complete and compliant.
  • Enter and maintain participant information within the electronic health record system.
  • Educate participants and caregivers regarding clinic services and respond to general inquiries.
  • Maintain exam room cleanliness and turnover between visits in accordance with infection control standards.
  • Assist with venipuncture and specimen collection when appropriately certified.
  • Communicate participant concerns or unusual findings to appropriate clinical staff.
  • Support orientation and training of new staff as assigned.
  • Participate in required compliance training, staff meetings, and emergency preparedness activities.
  • Maintain confidentiality of participant information and organizational data.
  • Promote a safe, respectful, and professional work environment.
  • Perform other related duties as assigned.
Qualifications Knowledge and Critical Skills

Knowledge of:

  • Principles, practices, and techniques applicable to Certified Nursing Assistants or Medical Assistants.
  • Infection control and prevention standards.
  • Safety practices within a healthcare environment.

Skills and Abilities:

  • Strong interpersonal and communication skills with participants, families, and healthcare professionals.
  • Excellent customer service orientation.
  • Ability to manage challenging behaviors calmly and effectively.
  • Ability to respond appropriately in emergency situations.
  • Ability to maintain strict confidentiality.
  • Strong organizational skills in a fast-paced clinical environment.
  • Basic computer proficiency and ability to utilize electronic health record systems.
  • Ability to collaborate effectively within an interdisciplinary team model.
Experience
  • Minimum of one (1) year of experience working with frail or elderly populations; candidates with less than one year may qualify if additional job-specific training is completed upon hire.
  • Two (2) years of experience as a Certified Nursing Assistant (CNA) or Medical Assistant (MA) preferred.
  • Demonstrated competency in required job skills prior to independent work performance.
License / Certification
  • Current CNA certification, completion of a state-approved Home Health Aide training program, or Medical Assistant certification as required by applicable state regulations.
  • Current CPR certification from a nationally recognized provider.
  • Valid state driver’s license with an acceptable driving record.
  • Proof of current automobile insurance (if travel is required).
  • Current tuberculosis screening and required immunizations in accordance with healthcare regulations.
  • Successful completion of required background screening in accordance with state regulations.
  • Phlebotomy certification preferred.
Education
  • High school diploma or GED equivalent preferred.
  • Equivalent combination of education and experience sufficient to successfully perform the essential functions may be considered.
Other Requirements
  • Reliable transportation for travel between participant locations, if required.
  • Ability to meet medical clearance requirements for communicable diseases prior to direct participant contact.
  • Ability to comply with all federal, state, and local regulatory requirements.
