Licensed Practical Nurse (LPN) – 1:1 Home Care

Location: Powder Springs, GA
Schedule:

• Monday–Friday: 9:00 AM – 3:00 PM
• Saturday: 9:00 AM – 2:00 PM
  Position Type: Day Shift, Ongoing Case

About the Case

We are seeking a reliable and clinically strong Licensed Practical Nurse (LPN) to provide 1:1 skilled nursing care for a client in Powder Springs. A CNA is already assigned to support all ADLs, allowing the nurse to focus on assessment, medication administration, and clinical oversight.

Primary Responsibilities

• Perform ongoing nursing assessments and monitor changes in condition
• Administer medications safely and accurately per provider orders
• Document all care in accordance with agency standards
• Communicate promptly with the care team regarding clinical updates
• Collaborate with the CNA to ensure coordinated, patientcentered care
• Maintain a safe, clean, and therapeutic environment
• Support the client during daily routines, appointments, or outings as needed
• Follow established care plan and report deviations to supervising RN/Provider

• Arrive on time and remain on site for the full scheduled shift
• Demonstrate strong clinical judgment and professionalism
• Maintain clear, respectful communication with family and staff
• Uphold all agency policies, infectioncontrol standards, and documentation requirements
• Provide compassionate, patientfocused care at all times

Required Qualifications

• Active Georgia LPN license
• Reliable transportation
• Strong assessment and medicationadministration skills
• Ability to work independently in a 1:1 home setting
• Current CPR certification
• Minimum 1–2 years of nursing experience preferred
