Minimum Requirements:
- Active Pharmacist license for Ohio required.
- Experience working as a Pharmacist. Minimum 6 months preferred.
- Experience with hospital pharmacy operations, including inpatient medication distribution, unit dose systems, and controlled substance management.
- Experience preparing and labeling parenteral and sterile products and familiarity with sterile compounding procedures.
- Familiarity with pharmacy inventory processes, procurement procedures, and medication return/recall handling.
- Experience with medication reconciliation for discharge medication lists.
- Clinical knowledge in antimicrobial stewardship, interpretation of culture results, and management of common infectious indications (UTI, pneumonia, SSTI) preferred.
- Experience with pharmacokinetic dosing management and anticoagulation monitoring preferred.
- EPIC electronic medical record experience preferred.
- Strong communication skills for obtaining home medication histories and providing discharge counseling.

- Facility Type: Hospital.
- Shift: Day (5x8).
- Duration: 13 week assignment.
- Floating: Candidate may be required to float to other affiliated facilities as needed.

Additional Information:
Under the general supervision of the Director. Responsibilities include maintaining adequate drug supply, procurement and inventory control, sterile and non-sterile preparation, dispensing and documentation, controlled substance accountability, medication reconciliation, clinical consultation, participation in Pharmacy & Therapeutics and quality programs, and staff education. Maintain confidentiality and comply with federal and state pharmacy laws and facility policies. Performs other duties as assigned.
