Minimum Requirements:
- Active Pharmacist license in the state of Ohio required.
- Experience in hospital pharmacy practice required. Experience with inpatient medication distribution, unit-dose systems, and sterile compounding expected.
- Experience with EPIC or similar electronic medical record systems required.
- Knowledge and experience in medication reconciliation for discharge medication lists required.
- Competence in antimicrobial stewardship, including interpretation of culture results and making therapy recommendations, preferred.
- Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring preferred.
- Familiarity with controlled substance handling, documentation, and regulatory compliance required.
- Ability to prepare, label, and validate sterile and parenteral products; competency assessment experience preferred.
- Effective communication skills for obtaining prior-to-admission medication histories and providing discharge counseling required.
- Compliance with federal and state pharmacy laws and hospital formulary processes required.
- Minimum 6 months of pharmacist experience preferred.

Standard Job Description:
We are seeking a licensed pharmacist to provide safe medication distribution and clinical pharmacy services in a hospital setting. Under the general supervision of the Director, the pharmacist will follow the organizations core values and safety priorities. Responsibilities include maintaining adequate drug supply and inventory, procurement of medications, removal and documentation of outdated or recalled drugs, and maintaining emergency drug stocks. The pharmacist will prepare, dispense, and properly label medications, including parenteral and sterile products, and ensure staff competency in sterile preparations.
Additional responsibilities include maintaining accurate pharmacy records, controlled substance accountability, conducting monthly inspections of pharmacy and medication storage areas, supporting the development and maintenance of the hospital formulary, and participating in Pharmacy and Therapeutics Committee activities. The pharmacist will perform medication reconciliation, provide patient discharge counseling, and participate in medication use evaluations and quality assurance activities. The role includes developing and delivering education to pharmacy staff and other healthcare professionals, supporting antimicrobial stewardship activities, monitoring drug therapy regimens for interactions and adverse reactions, and documenting clinical interventions.
The pharmacist will maintain confidentiality of patient and medical staff information, perform billing functions as required, support JCAHO accreditation efforts, and promote continuous quality improvement. Other duties as assigned.
