Registered Nurse Case Manager

URGENT Opening for Hybrid Case Manager- Registered Nurse at an insurance company in Washington, DC!!!! Interested candidates should have recent Case management/discharge planning experience in an acute care or insurance setting.

• 13+weeks, M-F, 8-4:30p
• No holidays or weekends (4 days in office, 1 day work from home)

Requirements of the CM Registered Nurse:

• DC Registered Nurse License
• Current Insurance Case Management experience required
• Cerner/Allscripts experience preferred
• Local resource experience preferred

Duties: This Complex Case Manager provides a service available to enrollees who have complicated medical conditions and need help arranging the services they need. Complex Case Managers work closely with doctors and other health care providers. They work closely with enrollees and or their representative(s) to assist with arranging doctor visits, medical equipment, therapy and other services to help enrollees regain optimum health or improved functional capacity. Will eventually be visiting client homes to assist with care plans of up to 2 days a week.
