Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Wisconsin Physical Therapy license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Must have at least 12 months of Home Health experience within the past 3 years (client requirement).
- Experience with HCHB (HomeCare HomeBase) EMR required (client requirement).
- Ability to travel within the assigned territory, including Minocqua area and Lac Du Flambeau (client requirement).

- Shift Name: Days.
- Shift Details: Days, 8 hours, Monday through Friday.
- Typical Schedule: 8:00 AM5:00 PM, MondayFriday.
