This facility is seeking a Dentist for locum tenens support as they look to fill a current need.

Details and requirements for this opportunity:

· Schedule: 4/6 6:30a-5p

· Setting: Clinic

· Procedures/Cases: Fillings, Orthodontics - Invisalign Certified (not required), Crowns, Bridges, Restore Implants, Cosmetic Dentistry. Number of dental assistants: 1-2 per dentist. Number of hygenists: 1

· Credentialing Timeframe: 1-2 days

· Requirements: State license, Drug Enforcement Administration license (DEA), Cardiopulmonary Resuscitation/Basic Life Support (CPR/BLS), National Provider Identifier (NPI #), Certificate of Insurance (COI), Client application.
