Job Summary and Qualifications The PGY1 Pharmacy Resident is responsible for providing complete pharmacy services in coordination and cooperation with department professional and support staff, consistent with department policies and procedures for operation and clinical practice, meeting all the requirements and obligations of pharmacists on staff. This includes monitoring patient medication regimens and providing appropriate interventions. The PGY1 Pharmacy Resident actively participates in rotation activities, including: team meetings, rounds, and other interdisciplinary conferences/meetings that occur.- Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
- Accurately compound and dispense pharmaceuticals.
- Comply with facility policy and procedures and regulatory requirements.
- Monitor drug therapy regiments for contraindications, drug-drug interactions, drug-food interaction, allergies and appropriateness of drug and dose. Provide clinical consultation and clarification to practitioners.
- Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- Sustain the hospital formulary, minimize nonformulary procurements, utilize therapeutic interchange and promote rational drug therapy selection.
- Attend and actively participate in multidisciplinary and bedside rounds.
- Complete drug utilization reviews to promote rational drug therapy and implement improvements identified.
- Provide accurate and timely medication information to healthcare staff and patients.
- Document all clinical activities and interventions accurately and completely Support and participates in the goals of the organization's quality improvement, risk management and patient safety program to include, but not limited to, National Patient Safety Goals, Core Measures and HCAHPS medication-related initiatives.
- Maintain professional competency sufficient to meet current and future needs of the facility.
- Participate with various nursing and medical staff committees as necessary.
- Provide in-services to pharmacy, nursing, and/or medical staff.
- Practice and adhere to the HCA "Code of Conduct" philosophy and "Mission and Value Statement".
- Serve as a staff pharmacist one half-day or evening per week from September through November, then one day or evening per week January through June and every third weekend (Saturday and Sunday), and is accountable for the preparation and distribution of all medications to patient care areas, and providing clinical pharmacy services to both inpatient and outpatient physicians, and other hospital personnel.
- Provide prescription services to inpatient/outpatients
- Checks unit dose medication for appropriate drug, dose, quantity, and packaging integrity
- Facilitates availability of first doses and specialty items
- Maintains adequate medication supplies
- Maintains and enforces the documentation and security of narcotic supplies
- Checks medication orders for therapeutic appropriateness
- Checks transcription of medication orders
- Update medications on administrative records
- Provides pharmacokinetic monitoring of patients receiving specific drugs and ensures that serum concentrations are drawn
- Monitors parental nutrition
- Observes for adverse drug reactions
- Documents all medication incident reports
- Oversee deliveries to patient care floors
- Compounds IV medications, including chemotherapy medications; and maintains patient confidentiality.
- Complete the University of North Texas System College of Pharmacy Training Certificate and course co-directorship.
- Successfully design, complete IRB submission for approval/exemption, carry out, and develop a manuscript of a major project to be determined and approved by the Residency Program Director (RPD) within the first 2 months of the residency start date. The objective is to present a poster at the ASHP Midyear Clinical Meeting. A manuscript will be developed and completed by the end of the residency year, and submission for publication is required.
