Minimum Requirements:
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA
- Active State License is Required to Start the Position
- BLS Certification May Be Required from AHA or ARC
- Experience in Home Health is required
- Experience with HCHB/Oasis is preferred
- Ability to manage a productivity expectation of 30 points/week
- Experience with adult and geriatric populations is preferred
