Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care and hospice care of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions. Education Qualifications:
- BSN or ADN degree from an accredited school or college of nursing is required
- East Market: Minimum of 1 year clinical nursing experience
This position is casual.
Licensure/Certification Qualifications:- Current nursing licensure in state (s) of employment. May be required to hold multiple state licensures, depending upon work location
- Basic Cardiac Life Support (BCLS) certification or ability to become certified within 1 month from date of hire
- Possession of a valid motor vehicle operator's license as verified through appropriate licensing agency
- Proof of current motor vehicle insurance
- Current RN license in the state of Minnesota upon hire. Ability to obtain RN license in Wisconsin within 3 months of hire
- At least 2 years of nursing experience, with a preference for experience in hospice, palliative care, home health care, or Critical Care.
- Recent Hospice experience within the last 5 years.
- Certification in Hospice and Palliative Nursing (CHPN) or willingness to obtain certification within a specified timeframe.
- Strong clinical skills in pain management and symptom control.
- Excellent communication and interpersonal skills to interact effectively with patients, families, and multidisciplinary team members.
- Ability to work independently and manage a flexible schedule.
- Competence in electronic health record (EHR) systems and other computer applications.
- Compassionate and empathetic demeanor.
- Strong organizational and time-management skills.
- Emotional resilience and the ability to handle stressful situations
- Ability to travel to patients homes and provide care in varying environments.
- Physical stamina to perform duties such as lifting, moving patients, and providing direct patient care.
- Possession of a valid motor vehicle operators license as verified through appropriate licensing agency.
- Proof of current motor vehicle insurance.
- Knowledge of Medicare, Medicaid, and other state and federal regulations related to hospice care.
