About the role The Diabetic Health Coach delivers telephonic health coaching to eligible members with Diabetes. The primary focus of this role is to engage members in behavior-change coaching that supports diabetes self-management, improved health outcomes, and sustained engagement in their care. The Diabetic Health Coach builds collaborative coaching relationship with members, using motivational interviewing to provide education, encouragement, and accountability as members work toward individualized health goals. Location: This position is located at our Dublin, OH campus. What you'll do (Essential Responsibilities) Member Engagement & Coaching Complete outbound calls to engage with participants that are eligible for program enrollment and to re-engage unresponsive program participants in Diabetes Program Utilize evidence based coaching skills, works with members to set health related goals and develop and implement effective strategies to reach those goals Provide coaching and education related to the following topics: blood glucose monitoring and interpretation, nutrition and physical activity, medication adherence and self-care routines, stress management, sleep, tobacco cessation, and social determinates of health Assess health, environment, nutrition, and behavioral areas of concerns using a variety of assessments, and in response to assessments, coach and problem solve with member to identify and address specific goal(s) to support health and behavior change Manages time to ensure that appointments are kept and appropriate follow-up is done in a timely manner Answer inquiries regarding claims, benefits, eligibility, provider searches and other health-related inquiries, as well as following up with members to ensure their needs have been met. Apply motivational interviewing technique when engaging with members telephonically Care Coordination & Navigation Reinforce provider care plans and evidence-based guidelines (e.g., ADA Standards) Identify gaps in care (A1c testing, kidney testing, eye exams etc.) and encourage and coordinate follow-up using available Plan benefit design options (e.g., biometric vendors, virtual care, primary care) Collaborate with integrated care team (internal and external care teams) Guide members and providers through the cost and complexity of healthcare. Assures that all member referrals are completed on a timely basis and documented according to client procedures. Identify and escalate issues to appropriate team members for resolution Program Execution & Documentation Utilize Quantum Health's systems and technology to accurately Develop individualized coaching plans with measurable goals Document coaching interactions, goals, and progress Track engagement, assessment, coaching plans, biometrics, adherence, and outcome metrics Document administrative details of active and non-active cases Support program reporting and quality improvement initiatives All other duties as assigned. What you'll bring (Qualifications) Education: Bachelor's degree in a health-related field e.g., nutrition, dietetics, public health, health sciences, exercise science, etc.) Licensure /Credentials : Registered Dietitian (RD/RDN) credential required; education requirements aligned with CDR standards. Current and active state licensure as applicable. Experience: Minimum of two years of experience as a health coach Knowledge of current health issues and behavior-change strategies Empathetic, member-centered communication. Excellent listening and interpersonal communication skills to identify critical core competencies based on success factors and organizational environment.
