How youll make an impact in this role
  • Reduce Readmissions: Oversee 90-day post-discharge monitoring using "teach-back" methods to ensure patients understand their discharge instructions and avoid unnecessary emergency visits and readmissions.
  • Streamline Transitions: Eliminate barriers to care by scheduling follow-up appointments, arranging transportation, and ensuring patients have timely access to community resources.
  • Enhance Patient Experience & Safety: Educate patients and families about medication management and symptom monitoring, empowering them to manage conditions at home and fostering trust in the healthcare system.
  • Promote Cost Efficiency: Ensure timely and appropriate follow-up care to reduce avoidable hospital usage and overall healthcare costs.
  • Coordinate Multidisciplinary Care:Serve as a central communication hub between healthcare providers to maintain consistent care plans.
What minimum requirements youll need

Licensure / Certification / Registration:

  • Licensed Registered Nurse obtained prior to hire date or job transfer date required.
  • Registered Nurse credentialed from the Oklahoma Board of Nursing required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional preferences we're seeking
  • Experience as an RN Care Manager, RN Hospice, RN Home Health, RN Patient Navigator, or a combination of these.
  • Empathy & Compassion: Capable of offering support during challenging transitions.
  • Strong Communication: Excellent at explaining information verbally and in writing, helping patients and coordinating with teams.
  • Critical Thinking: Ability to identify and solve obstacles to patient care.
  • Tech Savvy: Proficient with Electronic Health Records (EHR) and other platforms
  • Knowledge: Familiarity with healthcare systems, insurance plans (like Medicare/Medicaid), and community resources.
  • This position does require travel throughout NE OK 30%
