We are seeking a dedicated Speech-Language Pathologist (SLP) to join our close-knit team. If you are passionate about one-on-one home-based therapy, helping patients overcome communication and swallowing challenges, and making a meaningful difference every day, this is your opportunity!
Key Responsibilities
- Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders in their home environment.
- Develop and implement individualized treatment plans to address conditions such as stroke recovery, traumatic brain injury, neurological disorders, developmental delays, dementia, dysphagia (swallowing difficulties), aphasia, dysarthria, and more.
- Provide training and education to patients, families, and caregivers on safe swallowing techniques, augmentative communication strategies, home exercise programs, and compensatory methods.
- Document assessments, interventions, progress notes, and outcomes accurately and in compliance with agency, Medicare, and regulatory standards.
- Collaborate with the interdisciplinary care team (including PT, OT, RN, social workers) to deliver holistic, patient-centered care.
- Promote safety, independence, and improved communication/quality of life during home visits across the community.
- Adhere to infection control, HIPAA, and all agency policies.
Qualifications
- Master's degree in Speech-Language Pathology from an accredited program.
- Current, active Speech-Language Pathologist license/certification in the state of practice (Washington; multi-state a plus).
- Certificate of Clinical Competence (CCC) from ASHA preferred; Clinical Fellows (CFs) considered with strong supervision support.
- Valid driver's license, reliable transportation, and current auto insurance (required for home visits).
- Home health experience preferred, but new graduates or those with relevant clinical experience are encouraged to apply.
- CPR certification required.
- Strong interpersonal, communication, and organizational skills with a compassionate, patient-centered approach.
