Our OASIS Nurse (RN) utilize theirextensive education and training to provide personalized care to our patientsin a rewarding environment that promotes autonomy.
Job DescriptionPerform comprehensive OASISassessments to establish an individualized care plan.
Facilitate communicationbetween the patient and the interdisciplinary team.
Develop and maintain atherapeutic relationship by providing education and promoting self-advocacy andindependence.
Utilize state-of-the-arttechnology to document skilled services provided.
Qualifications
Registered Nurse with ValidCT RN license
One year of RN experience;Homecare experience preferred.
Valid drivers license andreliable transportation required.
Computer proficiencyrequired.
