Job Description
TLC Nursing Associates, Inc. is seeking a dedicated Registered Nurse (RN) Home Health to provide skilled nursing care to patients in their homes. The RN will assess patient needs, implement individualized care plans, and work collaboratively with healthcare teams to support patient recovery and well-being in a home setting.
Responsibilities- Deliver comprehensive nursing care to patients in their homes
- Conduct patient assessments and develop individualized care plans
- Administer medications, IV therapy, wound care, and other treatments
- Educate patients and families on disease management, home care techniques, and medication adherence
- Monitor patient progress and update care plans as needed
- Coordinate with physicians, therapists, and other healthcare providers
- Ensure compliance with home health regulations and standards
- Maintain accurate and timely documentation
- Active RN license in applicable state(s)
- Minimum 12 years of nursing experience (home health experience preferred)
- Certification in Basic Life Support (BLS) required
- Strong ability to work independently in a home setting
- Excellent clinical assessment, critical thinking, and communication skills
- Valid drivers license and reliable transportation
