Job Description


  • Coaches recovering people in the independent use of community supports and resources.
  • Facilitates the completion of Recovery Plans that are individualized, strength-based and recovery focused based upon the participants’ hopes, dreams and aspirations. 
  • Acts as mentor, motivator, skill builder and educator to individuals in relationship to their Recovery plans.  Assists individuals in acquiring the knowledge, skill and abilities strategies to identify, choose, reach and keep their life goals.  
  • Assist PIR in identifying particular challenges, circumstance, risk factors and other potential challenges to their recovery plan and create strategies to address these, ie WRAP, crisis plans, etc. Help PIR identify and overcome obstacles and fears
  • Facilitates and supports PIR in their identification of social networks outside the program.
  • Helps PIR identify and create an interest in community life; teaches skills for living in the community by identifying the array of steps needed to live in the community.
  • Identifies natural supports that already exist in the person’s life and enhances that support. With permission, engages family members and other key individuals to participate in and support an individual’s recovery.
  • Accesses and refers individuals/family to community resources for social support, medical care, specialized service or treatment, peer run recovery groups, faith based organizations, treatment, education, employment and basic needs (food, clothing, shelter, financial).
  • In partnership with administrative staff and PIR, identifies potential partnerships that support PIR and the program.
  • Provides the necessary supports to individuals as they experience stigmatizing attitudes, beliefs and behaviors.
  • Advocates for and on behalf of PIR with the service, treatment team, and the agency at large.
  • Encourages peer leadership, peer involvement, peer culture, peer support and inclusion in all aspects of the program.
  • Participates in staff trainings, team meetings, and consultation meetings as organized within the program.
  • Promptly (within 24 hours) completes documentation in a manner that adheres to required standards, regulations and guidelines of event.
  • Conducts follow-up and outreach as appropriate to engage or support members in their treatment or recovery. This includes collaboration with member’s identified support network.  Documents these efforts in a timely manner.
  • Meets agency, regulatory, or licensing standards for compliance with training requirements.  Ensures that employment record is up-to-date by submitting and/or updating documents as they expire or as requested. 
  • Keeps abreast of changes in the field.
  • Completes chart reviews in accordance with program guidelines to ensure 100% compliance with documentation requirements.                         
  • Assists individuals with personal hygiene or other aspects of daily living as needed. 

 

       Qualifications/Education:

  • Bachelor’s degree and at least 2 years of mental health direct care experience preferred.  
  • Must become certified in Psychosocial Rehabilitation within specified time frames and maintain certification once obtained.  
  • General knowledge of Principles of Recovery and Psychosocial Rehabilitation; experience working in a recovery oriented setting preferred.
  • Demonstrated interest in and ability to work with diverse populations and cultural backgrounds. 
  • Knowledge of/willingness to learn and use Philadelphia public transit system required.
  • Local travel in the community with small groups of program participants using various modes of public transportation required.
  • Ability to work a flexible schedule which may include evenings and/or weekends. §        
  • Basic knowledge of personal computers.
