Under direct supervision of the Clinic Manager, with indirect supervision by CNO/Clinic Director, the Nurse Navigator will provide direct patient care and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with clinical research office, increase patient and provider satisfaction, maintain point of contact with providers and serve as the first point of contact for patients and families.
How you'll contribute
A Registered Nurse (RN) who excels in this role:
- Provides direct patient nursing care for specified patients, including, service lines available, but not limited to ORHC, appropriate supportive care as prescribed.
- Performs nursing assessments and triage of patient care needs for new and ongoing patients.
- Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
- Work with physician and clinical ancillary offices to facilitate, coordinate and ensure patient access to service lines
- Central point of contact for all navigated patients; including interaction with medical, nursing, ancillary services, and when appropriate the nursing supervisor/coordinator.
- Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public.
- Collect and report data from patient on barriers, health care, medications, prior treatments, etc.
- Track order completion and follow up. Monitor patient compliance with patient treatment plan.
- Assist patients with referrals to outside resources and care coordination as needed.
- May attend and refer patients to ancillary services recommendations.
- May participate in the orientation, training and evaluation of new staff.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control standards.
- Collaborates with medical providers, patient care staff and clinic management in the planning and implementation of patient and staff education.
- Designs or participates in the creation of health education materials.

What we're looking for
Applicants should have a current state RN license and possess a bachelor's degree from an accredited nursing school. Additional requirements include:
- Basic Life Support (BLS)
- At least 3 years of experience directly related to the duties and responsibilities specified
- Master's degree in nursing preferred but not required
