Hope & Healing is proud to be the first certified RISE provider in the state of Kentucky. RISE stands for Recovery, Independence, Support, and Engagement. It is a NEW program in KY that provides home and community-based services for adults with serious mental illness (SMI). This program will completely change the landscape of mental health care! People with SMI will now have access to services that are needed and long overdue. These services will be paid under the 1915(i) RISE Initiative. There are no waiting lists or slots, if you qualify then you will be provided the services you need. We provide all 9 RISE services, including Supervised Residential Care, Supported Employment, Respite for Caregivers, Transportation, Medication Management, Supported Education, Housing & Tenancy Supports, In-Home Independent Living Supports, and Assistive Technology. Each participant will also have a Case Manager provided by an agency of their choice. Hope & Healing is excited about these LIFE-CHANGING services.
