Upper Great Lakes Family Health Center (UGL) established in 2009, is a private not-for-profit health center that operates 9 service delivery locations serving Houghton, Iron, Marquette, Menominee and Ontonagon Counties. Services (onsite and through referral) include primary care, oral health care and behavioral health services, substance abuse counseling, enabling services, specialty care/services, education and comprehensive outreach.

UGL is one of 42 Michigan Health Center organizations, and is recognized as a Federally Qualified Health Center (FQHC). FQHC’s receive grants under Section 330 of the Public Health Service Act, and qualify for enhanced reimbursement from the Centers for Medicare and Medicaid. FQHC’s cater to the underserved population by offering comprehensive services, an ongoing quality assurance program and for qualified individuals, discounted services. A top priority of a FQHC is to provide high quality primary care to everyone, regardless of their ability to pay.

Our health care team delivers services based on the Patient Centered Medical Home (PCMH) model of care. PCMH is a model of healthcare based on an ongoing, personal relationship between a patient, provider and the patient’s care team. Whatever the medical needs – primary or secondary, preventive care, acute care, chronic care, behavioral health, or end-of-life care – the patient has a single, trusted provider and care team, through which continuous, comprehensive and integrated care is provided.

When specialty care is needed the provider and care team will personally manage, facilitate and coordinate care with appropriate qualified professionals, such as hospitals, nursing homes, pharmacies, behavioral health practitioners and related community resources.
