Stephen F. Austin Community Health Network, dba Community Health Network (CHN) was established in 2008 to provide accessible and affordable healthcare for people of Brazoria County. That year, the Center had one health care provider with three exam rooms providing care at the Adoue Family Health Center. Since that time, the Center has grown substantially, adding a dental department, women’s health care, behavioral health counselors, and psychiatry. In 2013, CHN launched a second site located in Freeport, Texas. The Freeport Community Health Center provides adult medicine, women’s health, obstetrics & gynecology, behavioral health and general dentistry services.



Due to growth and demand for services in Alvin, CHN has opened four additional sites: the Women and Children’s Health Center, Callaway Behavioral Health Center where we provide psychiatry, group & individual counseling services; the Pearland Family Health Center; and the Children’s Clinic-League City. The Center’s newest location is our Mobile Clinic that is equipped with a medical and dental exam room. The Mobile Clinic will enable CHN to travel throughout the County delivering care in more remote areas in partnership with community partners. In the Center’s first year of service 1,025 patients received care, In 2017 the number of people that received care was 13,350.
