Tri-County Care is a newly designated Care Coordination Organization Health Home- CCO/HH (one of only seven designated by New York State). Tri-County Care CCO, is sponsored by the New York State Hamaspik Association Inc.( NYSHA) , NYSHA is proud of its rich heritage working with the I-DD community for over two decades. NYSHA services, funded by the Office for People with Developmental Disabilities or (OPWDD), include: MSC, residential and respite care, community habilitation, family supports, day services, EMODs, ABA, Article 16, self-directed care services and vocational, as well as, job coaching services.
