City Choice Home Care provides a quality home care services with most qualified and companionate staff available. We provide Skilled Nursing Services, Home Health Aide (HHA) Personal Care Aide (PCA) services and CDPAP services within the NYC boroughs and Westchester County.
Our specialty in providing seniors with caring, professional home care combined with personalized attention and exceptional customer services. Our nursing staff meets with patients and their family members at home, and an individualized plan of care for the patient is developed in collaboration with the patient’s physicians.
Our home health aides are given in service training covering nutrition, fall prevention, diabetes, pain management, hospice care and many other topics. City Choice is available 24 hours a day, 7 days a week to meet our patient’s needs. We are a provider for numerous certified home care agencies, managed long term care programs, and hospice programs, and service many private pay patients as well.
