SUMMARY: The Licensed Behavioral Health Provider provides individuals and families with the psychosocial support necessary to cope with chronic, acute or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare. Due to client base demographics, English/Spanish bilingual fluency is preferred. This position is an integral part of the patient-centered medical home care team and works collaboratively with others to support optimal patient outcomes. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Evaluate and assess patients to determine their needs and the support options that can help them • Refer patient or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education. • Provide patients in individual sessions to assist in recovery from behavioral health issues, including mild to moderate behavioral health diagnoses and substance use disorders. • Collaborating with other care providers to coordinate services through an integrated care approach. • Utilize consultation data and behavioral health experience to plan and coordinate patient care and rehabilitation, following though to ensure service efficacy. • Modify treatment plans to comply with changes in clients’ status. • Monitor, evaluate and record patient progress according to measurable goals described in treatment and care plan.
