Minimum Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Illinois state physical therapy license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Experience in outpatient physical therapy required.
- Experience with patient evaluation, goal writing, and developing functional treatment programs.
- Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry.
- Must adhere to state Practice Act, maintain professional licensure in good standing, and meet state-mandated continuing education requirements.
- Willingness to float to other clinic locations as needed.
Preferred Qualifications
- Experience supervising students and participating in clinical education programs.
- Prior experience in performance improvement initiatives and familiarity with competency programs for direct care providers.
- Demonstrated safe patient handling and materials handling practices.
Assignment Details
- Shift pattern: 5x8 schedule.
- Floating required to other clinic locations as needed.
