Physical Therapist - Travel Contract

Join a traveling Physical Therapist role in Chicago, IL at an outpatient clinic where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location. The role involves evaluating assigned patients, developing and implementing individualized treatment plans, documenting care in compliance with regulatory and clinic standards, supervising discipline-specific therapy services, and participating in student education and professional development. The position requires adherence to safety procedures and the organizations code of conduct.

Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois state physical therapy license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience in outpatient physical therapy required.
• Experience with patient evaluation, goal writing, and developing functional treatment programs.
• Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry.
• Must adhere to state Practice Act, maintain professional licensure in good standing, and meet state-mandated continuing education requirements.
• Willingness to float to other clinic locations as needed.

Preferred Qualifications

• Experience supervising students and participating in clinical education programs.
• Prior experience in performance improvement initiatives and familiarity with competency programs for direct care providers.
• Demonstrated safe patient handling and materials handling practices.

Assignment Details

• Shift pattern: 5x8 schedule.
• Floating required to other clinic locations as needed.
