Minimum Qualifications:

  • Current pharmacist license in Pennsylvania required.
  • Experience in hospital pharmacy practice required.
  • Experience with sterile product preparation, parenteral medications, and labeling.
  • Knowledge of federal and state pharmacy laws, controlled substance documentation, and inventory management.
  • Experience with medication reconciliation and discharge medication counseling.
  • Clinical experience in antimicrobial stewardship, interpretation of culture results, and making antimicrobial recommendations.
  • Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring.
  • EPIC experience required.
  • Strong written and verbal communication skills for gathering home medication lists and conducting discharge counseling.
  • Minimum 6 months of pharmacist experience preferred. (Preferred)

  • Shift pattern: 5x8 day schedule.

Impacting Patient Care

Provide pharmacy services that support patient and employee safety, including medication procurement, inventory control, sterile product preparation, controlled substance management, and medication-use evaluations. Participate in interdisciplinary teams, Pharmacy and Therapeutics committee activities, quality assurance programs, and staff training. Perform medication reconciliation, discharge counseling, and clinical monitoring to optimize patient outcomes.
