RN: Case Manager

Job Title - RN: Case Manager

Shift: Monday to Friday, 8 AM to 5 PM (Weekend on-call rotation)

Productivity: 30 points per week

License Required: RN License in Indiana or Compact License accepted

Mileage Reimbursement: $0.46 per mile

Coverage Areas: Clark, Floyd, Harrison, Crawford, Scott, Washington, Jefferson, and part of Orange County. May also help in other areas for PTO coverage, call-ins, and admissions.

Required Skills: Wound care, IV care, drain care, G-tube care, and patient education. HCHB experience preferred.

Primary Responsibilities:

• Complete assessments for high-risk patients
• Create and follow individualized care plans
• Coordinate with doctors, social workers, and care teams
• Support patients with non-medical needs like housing and transportation
• Manage care transitions and discharge planning
• Keep accurate patient and care records
• Promote health and prevent illness
• Protect patient privacy and confidentiality

Qualifications:

• Associate’s or Bachelor’s Degree in Nursing
• Active and unrestricted RN license
• Strong decision-making and medical assessment skills
• Case Management certification preferred
