• Schedule: Various June Weekdays, 8:30 a.m.-5:00 p.m.
• Patients Per Day: 22
• Practice Setting: Outpatient
• Scope: Standard Primary Care
• State Licensure: Active California License and Drug Enforcement Administration (DEA)
• Credentialing Timeframe: 2 Weeks
