Per Diem Nurse Practitioner - Grove City, Ohio

Provide essential healthcare services as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members in Columbiana County, Ohio. This per diem role offers flexible scheduling between Monday and Sunday, allowing you to create your own hours with a commitment of a minimum number of shifts on a weekly or bi-weekly basis. Ideal candidates will hold an Ohio Nurse Practitioner license and have experience in delivering patient-focused care.

Key Responsibilities:

• Perform annual home health risk assessments, including post-discharge visits, averaging approximately 45 minutes each.
• Assess patients prior and current medical and medication history.
• Educate patients on medical conditions and medication management.
• Identify gaps in care and recommend appropriate interventions.
• Note: Treatment plan creation and prescribing are not part of this role.

Qualifications:

• Licensed Nurse Practitioner in Ohio.
• Current Basic Life Support (BLS) certification.
• Board Certified through AANP or ANCC.
• Preferred minimum of 6 months recent NP experience.
• Master of Science in Nursing (MSN) from an accredited institution.
