Assertive Community Treatment (ACT) is provided by an interdisciplinary team that ensures service availability 24 hours a day, seven (7) days a week, with ages 18+ yrs. and is prepared to carry out a full range of treatment functions wherever and whenever needed. An individual is referred to the ACT team service when it has been determined that his/her needs are so pervasive and/or unpredictable that it is unlikely that they can be met effectively by other combinations of available community services, or in circumstances where other levels of outpatient care have not been successful to sustain stability in the community. Typically this service should be targeted to individuals who have serious and persistent mental illness or co-occurring disorders, multiple diagnoses, and the most complex and

expensive treatment needs.

Position Summary

The Registered Nurse position provides medical assistance to the clients served through NYAP including performing physical assessments and treating acute, minor illnesses as needed.

Responsibilities

The Registered Nurse will perform duties including, but not limited to:

Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies

Perform physical assessments on each client upon arrival and intake.

Identify abnormal findings and notify the client’s physician or consulting physicians.

Assure the need for medical treatment for illnesses and/or injuries are being met.

Assure the administration and implementation of physician orders, and when appropriate, educate the appropriate staff on administering medication and/or treatment as needed.

Assure all documentation and required record keeping is completed in a timely manner.

Assess minor illnesses and administer non-prescription medications per approved Standing Orders Policy.

Schedule and coordinate examinations by the consulting physician for each individual.

Coordinate care with the Team to assist in medical, physical, and mental wellbeing of individuals served.

Assure the program’s compliance with Medical Policies and Procedures as well as Standing Orders

Provide in-service training and continued education to staff on health-care related needs of individuals served, CPR, and First Aid.

Reviews medical and clinical charts, and other personal documentation to monitor physical and mental health progress.

Consults with other professionals regarding treatment planning and medical recommendations.

Monitors medication compliance and any possible side effects reported by individuals served.

Maintain Red Cross CPR/First Aid Trainer’s Certification.

Other duties as assigned by Program Director or other direct supervisor.

Minimum Qualifications

Must be a Registered Nurse with valid licensure.

Nursing experience working with adults with behavioral needs is preferred.

Must be available for on-call phone consultations as needed.

Must be able to maintain compliance with attending agency required training programs.

Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
