This facility is seeking a Dentist for locum tenens support as they look to fill a current need.

Details and requirements for this opportunity:

· Schedule: Monday 8:00am-5:30pm, Tuesday 7:30am-4:00pm (Green Bay Office) and Wednesday 7:30am-4:30pm, Thursday 8:00am-5:30pm (Appleton Office) - offices are 25 miles apart

· Setting: Clinic

· Procedures/Cases: Orthodontics - 98% of patients are adults/adolescents.

· Credentialing Timeframe: ~1-2 days

· Requirements: Active State License, Drug Enforcement Administration (DEA), Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), Controlled Substance Registration (CSR) and use of Dentrix Electronic Medical Record (EMR)
