Key Responsibilities: Conduct comprehensive nursing assessments within 8 hours of patient admission, forming individualized care plans tailored to rehabilitation needs. Collaborate with patients, families, and interdisciplinary teams to identify teaching goals and ensure consistent education throughout the rehabilitation process. Document all assessments, interventions, and patient responses accurately and promptly in medical records. Participate actively in patient rehabilitation programs, applying nursing processes focused on assessment, planning, implementation, and evaluation. Initiate and support discharge planning from admission, preparing patients and families for a safe transition. Communicate effectively with physicians, charge nurses, and team members regarding patient status and changes. Supervise and guide ancillary nursing staff, delegating responsibilities and providing leadership in problem-solving and prioritization. Maintain a safe environment for patients and staff by following safety protocols including safe patient handling, infection control, and workplace hazard reporting. Contribute positively to unit dynamics by supporting teamwork and efficient communication. Manage supplies and equipment to provide cost-effective care. Assume charge nurse responsibilities as needed post-orientation. Engage in ongoing professional development and education to enhance nursing skills. Qualifications: Current RN license with experience in Rehabilitation and Long-Term Care nursing. Strong clinical assessment and critical thinking skills.
