Minimum Qualifications
Bachelors degree with progressive experience or a Masters degree in Physical Therapy
Active Physical Therapist license in the State of Ohio
Working knowledge of home health Conditions of Participation preferred
Home healthcare experience is a plus
Current drivers license and reliable transportation
