Position SummaryThe Physical Therapist plays a vital role in helping patients regain strength, independence, and quality of life after illness, injury, or surgery. Guided by the Director of Nursing, the role creates personalized treatment plans and deliver hands-on, compassionate care using therapeutic exercises, mobility training, and specialized techniques.
Collaborates closely with physicians, nurses, and care coordinators to ensure seamless, high-quality care, while tracking and reporting progress to help patients meet their recovery goals.
Essential Functions- Conduct in-person evaluations and reassessments to develop individualized physical therapy treatment plans for home health patients.
- Collaborate with the interdisciplinary care team, including nurses, physicians, and case managers, to coordinate care planning and ensure 24-hour continuity of care.
- Implement treatment programs using evidence-based physical therapy techniques, and adjust plans based on patient progress and clinical judgment.
- Assess patient needs for durable medical equipment, prosthetics, and home safety modifications.
- Educate patients and families on therapy goals, home exercises, and safe mobility practices to promote independence.
- Supervise and review care provided by Physical Therapist Assistants (PTAs), including reviewing documentation, plans of care, and conducting competency evaluations.
- Accurately document all assessments, treatments, and patient education in accordance with organizational and regulatory standards.
- Participate in quality improvement initiatives, staff training, and professional development activities.
- Maintain compliance with clinical, legal, safety, and ethical standards in all aspects of care.
- Represent Alivia Care @ Home professionally and support the mission of delivering high-quality, patient-centered home-based care.
- Proficient in therapeutic modalities, including exercise, mobilization, manual therapy, and thermal/electrical stimulation techniques
- Skilled in restorative and rehabilitative strategies to improve function, reduce pain, and promote independence
- Strong clinical judgment, problem-solving, and decision-making abilities
- Effective communicator with active listening skills and the ability to educate patients, families, and care teams
- Comfortable working autonomously in patients' homes while collaborating within an interdisciplinary care team
- Knowledgeable in home safety risk factors and injury prevention techniques
- Able to provide functional training in self-care and mobility to patients and caregivers
- Enthusiastic about delivering care in a home health environment
- Proficient with Microsoft Office Suite and comfortable using CRM and home health documentation systems
- Bilingual in Spanish is a plus
- Minimum of 1 year of physical therapy experience preferred; new graduates are welcome to apply
- Licensed Physical Therapist in the State of Florida
- CPR or BLS certification
- Graduate of a CAPTE-accredited program or foreign equivalent recognized by APTA or U.S. regulatory standards
- Must meet educational requirements for Medicare reimbursement as outlined in Addendum
- Valid Florida driverโs license with acceptable driving record
- Reliable transportation
- Proof of current automobile insurance
- Level II AHCA background screening clearance
