Telehealth Primary Care Physician Role Overview:

The physician serves as a primary care physician within the VISN 21 Clinical Resource Hub (CRH). The physician will provide virtual primary care services to VA clinics facing staffing shortages within VISN-21, which comprises northern and central California, Nevada, and the Pacific Islands including Guam. The physician will work with an inter-professional team of nurses, pharmacists, and mental health providers to deliver team-based primary care. The position is remote; however, we do require that the provider travel to the individual clinic they are serving to deliver in-person care every quarter through VA-funded travel.

Responsibilities Include:

Veteran-Centered Care:

  • Deliver comprehensive primary care services via telehealth with an emphasis on health maintenance, disease prevention, patient education, and management of chronic illnesses.

  • Promote shared decision-making with veterans and their families to achieve optimal health outcomes.

Clinical Excellence:

  • Conduct virtual physical examinations and diagnose medical conditions.

  • Order and interpret diagnostic tests, including laboratory tests and imaging studies.

  • Develop individualized treatment plans and prescribe appropriate medications.

  • Conduct virtual primary care clinics and provide telehealth consultations as needed.

Quality Improvement:

  • Participate in continuous quality improvement projects to enhance the delivery of care.

  • Utilize various quality measurement tools to monitor and improve clinical performance and patient outcomes.

Clinical Leadership:

  • Serve as a mentor and role model for other healthcare providers within the primary care team.

  • Engage in educational activities, providing guidance and support to residents, students, and other trainees.

Healthcare Coordination:

  • Coordinate care for veterans involving specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery.

  • Collaborate with other healthcare providers to develop individualized treatment plans for veterans.
