At Wellspring Home Health Center, we are dedicated to providing high-quality, compassionate in-home care that helps patients achieve the highest level of independence and comfort in their own homes. We serve communities in the Tacoma/Pierce County and south King area in Washington with personalized, skilled services, including therapy, nursing, and support for seniors, those with disabilities, and individuals recovering from illness or injury.

We are currently seeking a motivated and caring Physical Therapy Assistant (PTA) to join our growing team. If you are passionate about making a real difference in patients' lives through one-on-one home-based care, we want to hear from you!

Key Responsibilities

  • Implement individualized physical therapy treatment plans developed by the supervising licensed Physical Therapist.
  • Assist patients with therapeutic exercises, manual techniques, functional training, gait training, transfer activities, and pain management strategies.
  • Teach patients and caregivers safe mobility techniques, use of adaptive equipment, and home exercise programs.
  • Document patient progress, treatment sessions, and changes in condition accurately and promptly in accordance with agency and regulatory standards.
  • Collaborate with the interdisciplinary team (PT, RN, OT, SLP, social workers) to ensure comprehensive, patient-centered care.
  • Provide care in patients' homes across the community, promoting safety, independence, and quality of life.
  • Maintain compliance with all state, federal, and agency policies, including infection control and HIPAA.

Qualifications

  • Associate's degree from an accredited Physical Therapist Assistant program.
  • Current, active Physical Therapist Assistant license/certification in the state of practice (Washington; multi-state licensure a plus).
  • Valid driver's license, reliable transportation, and current auto insurance (required for home visits).
  • Home health experience preferred, but new graduates with strong clinical rotations are encouraged to apply.
  • CPR certification required.
  • Excellent communication, interpersonal, and organizational skills.
  • Compassionate, patient-focused attitude with a commitment to treating every patient like family.
