Registered Dietitian - PACE Program

Location: Evansville, IN
Job Type: Full-Time
Salary Range: $70k - $73k and comprehensive benefits

Position Summary

We are seeking a Registered Dietitian to join a mission-driven PACE (Program of All-Inclusive Care for the Elderly) organization. This role is responsible for assessing participants' nutritional status, developing individualized nutrition interventions, and collaborating with the Interdisciplinary Team (IDT) to ensure comprehensive, participant-centered care.

The Registered Dietitian plays a critical role in supporting frail and elderly participants through nutrition counseling, care planning, and ongoing monitoring to promote health, independence, and quality of life.

Key Responsibilities
  • Conduct comprehensive nutritional assessments at enrollment and periodic reassessments per PACE regulations

  • Develop and update individualized nutrition care plans in collaboration with the interdisciplinary team

  • Provide nutrition counseling and education to participants, caregivers, and families

  • Monitor weight, hydration, intake, swallowing/chewing issues, mood changes, skin integrity, and drug-nutrient interactions

  • Recommend and coordinate nutrition-related interventions such as home-delivered meals and supplements

  • Communicate participant nutrition needs and recommendations to the IDT

  • Participate in interdisciplinary team meetings, family meetings, and care conferences

  • Incorporate cultural and ethnic preferences into individualized nutrition plans

  • Provide education and guidance to healthcare professionals and center staff

  • Participate in quality improvement initiatives and training programs

  • Maintain accurate, comprehensive documentation and follow infection control and safety protocols

  • Perform additional duties as assigned, including Food Service Supervisor responsibilities when applicable

Qualifications & Experience
  • Bachelor's degree in nutrition, dietetics, or related field from an accredited college

  • Registered Dietitian credential (passing the Commission on Dietetic Registration exam)

  • Minimum 1 year experience with elderly or frail populations (training provided if not present)

  • Minimum 2 years clinical nutrition experience preferred

  • ServSafe certification preferred

Skills & Competencies
  • Strong clinical assessment and care planning skills

  • Knowledge of geriatric nutrition, chronic disease management, and drug-nutrient interactions

  • Excellent communication and counseling skills

  • Ability to collaborate within an interdisciplinary care model

  • Strong documentation and organizational skills

  • Cultural competency and patient-centered approach to care
