Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• 2+ years current experience in Physical Therapy in Home Health
• Home Health and/or Hospice experience
• OASIS experience
• Permanent address must be included in submission file

• Facility Type: Home Health
• Shift: Days
• Shift Details: Monday - Friday; around 8/9am start, 40 hours per week
• Weekend Requirement: None
