What is Your Role? The Clinical Nursing Supervisor is responsible for assisting the Director of Clinical Services in ensuring that patient care is coordinated and managed in accordance with federal, state, and local regulations and standards, ensuring that each patient receives quality care. The RN Supervisor directs clinical personnel, promoting a positive, calm and effective work environment that contributes to teamwork. This position requires excellent communication skills with team members, residents and family members as well as a high level of patience and compassion for others.

Reviews case referrals. Reviews available patient information related to the case, including home visits, to determine hospice/home care needs. Assign appropriate hospice personnel to the case as needed. Conferences with the Medical Director regarding any questions about an individual's eligibility for services.

Reviews and evaluates each case through a variety of means, such as home visits, conferences, and records, and reviews the services provided by clinicians; discusses and provides impressions.

Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.

Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.

Assists case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

Participates in quarterly record reviews under the direction of the Director of Clinical Services and communicates findings and recommendations.

Assists in the screening and interviewing process of new associates and makes recommendations. Assists in the orientation of new associates.

Assists in assessing the health needs of the community and the development of plans/programs to meet needs. Participates in public relations and community activities that promote the Wesley Hospice and Life Enriching Communities.

Assists with evaluating quality assessment performance improvement (QAPI) plans and participates in QAPI teams and activities as needed.

Participates in State Survey.

Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.

Communicates with Doctor(s) families, and other health professionals.

We are seeking:

Must possess current RN license in the state of Ohio.

Diploma from accredited nursing school.

One plus year of hospice care experience, two years of acute care experience in an institutional setting, or 2 years of home care experience is strongly preferred.

Prior supervisory experience required.

Must possess a customer service background and orientation with demonstrated problem solving skills.

Microsoft Office proficiency
