MDS Coordinator, RN

We’re seeking a dedicated MDS Coordinator, RN to join Harrogate, a life-plan community located on the border of Toms River and Lakewood, New Jersey.

You’ll complete resident assessments, coordinate MDS documentation, and work closely with our interdisciplinary team to ensure high-quality care and regulatory compliance.

9am-5pm

What You’ll Do:

• Complete comprehensive resident assessments and timely MDS documentation
• Collaborate with care teams to develop individualized care plans
• Ensure compliance with federal and New Jersey state regulations
• Review clinical documentation for accuracy and HIPAA compliance
• Educate and support staff on MDS processes and documentation standards
• Participate in utilization review and interdisciplinary meetings

What We’re Looking For:

• Current and Valid New Jersey RN license
• Experience with MDS/PDPM process for Sub-Acute and LTC residents
• Must have experience operating Point Click Care for assessments and EMR
• Knowledge of State and Federal regulatory requirements
• Strong clinical knowledge, organizational skills, and attention to detail
• Participate in developing/updating/reviewing/completion of individualized Care Plans
• Excellent communication and teamwork skills
