Pathway Homes is a private, nonprofit striving to end homelessness by providing housing and supportive services for adults with mental illness, substance use and other co-occurring disorders in Northern Virginia. We are seeking a Substance Use Disorder Specialist with CSAC certification to join our Assertive Community Treatment (ACT) Team based in the Reston/Herndon areas of Fairfax County.
An ACT program is an interdisciplinary team that provides wrap around services 7 days per week to individuals in their homes and in the community. The goal of program is to decrease the utilization of ERs, in-patient hospitalizations and involvement with the legal system, by supporting individuals in developing effective healthy living skills.
The ACT Substance Use Disorder Specialist's essential duties & responsibilities:
< Provide direct services including case management, crisis intervention and teaching of independent living skills.
< Participates in the development & Implementation of individualized service plans for dually diagnosed clients.
< Provide education and integrated co-occurring substance use and mental health assessment as well a tracking stages of change and readiness for treatment.
< Use motivational interviewing and outreach techniques, cognitive behavioral approaches, and relapse prevention techniques to support individuals who have substance use concerns.
< Supports team members with identifying and delivering SUD care in alignment with integrated treatment models considering the interaction of mental illness and substance use and allowing for harm reduction and using stages of change to understand care delivery.
< Responds to crisis and takes appropriate action.
< Connects participants with needed community resources.
< Works collaboratively with interdisciplinary team. < Rotating on-call availability Schedule: Sunday-Thursday 9:30-6pm with on-call rotation Job Requirements: Bachelor's degree in Psychology, Social Work, Social Sciences, or related field and one year of experience in direct work with dually diagnosed individuals. CSAC certification. Must have valid drivers license and reliable vehicle. Must be able to pass a drivers record check and a criminal background check.
