The Community Paramedic is responsible for providing in-home and mobile healthcare services to patients of all ages in their homes. This role includes conducting health assessments, delivering care, offering chronic disease management, assisting with care coordination, and addressing social determinants of health. The Community Paramedic will collaborate with healthcare providers, Managed Care Organizations (MCOs), Clinical Research Organizations (CROs), Accountable Care Organizations (ACOs), Clinical Care Organizations (CCOs), and social service agencies to support improved health outcomes, reduce unscheduled care, and provide personalized clinical services. Care responsibilities also include advanced procedures such as medication administration, IV placement, blood draws, wound care, and setting up telemedicine technology for remote consultations.

Work Schedule Overview:

  • Weekday Hours:
    • Three weekdays (to be determined), each with an 8-hour shift from 9am to 5pm
  • Total Weekly Hours:
    • Less than 30 hours

This part-time role offers a flexible work schedule, focused on weekdays only. Currently structured to stay under 30 hours per week, there is potential for the position to expand into full-time as market demand grows, offering opportunities for career advancement and further responsibilities.

Job Duties and Responsibilities

Patient Assessment and Care Services:

  • Deliver direct patient services, including evaluation, screenings, education, treatment, and case management.
  • Conduct in-home health assessments, including vital signs, health screenings, and home safety evaluations.
  • Administer treatment based on established medical protocols, including advanced medication administration, IV therapy, and catheter placement. Provide patient education on chronic disease management, medication adherence, and preventive care.
  • Assist patients with post-hospital discharge care, including follow-ups to minimize ER visits and readmissions.
  • Manage and support patients with chronic conditions such as diabetes, asthma, COPD, hypertension, CHF, and behavioral health diagnoses.
  • Conduct point-of-care lab testing, blood cultures, wound evaluations, and geriatric assessments.
  • Collaborate with community resources, caregivers, and healthcare professionals to coordinate care and navigation services.
  • Maintain compliance with HIPAA, infection control protocols, and universal precautions.
  • Provide customized/personalized services as requested by partner organizations consistent with the IHCare+ protocol and Paramedic Scope of Practice

Medical Equipment and Technology:

  • Deploy and troubleshoot in-home monitoring equipment and tech packs for virtual physician interactions.
  • Monitor and maintain patient equipment and supplies.

Documentation and Reporting:

  • Document all patient interactions, assessments, care plans, and outcomes in electronic health records.

Community Outreach

  • Participate in community outreach programs, health promotion events, and disaster response efforts as needed.

Continuous Learning and Professional Development:

  • Participate in ongoing training, professional development, and continuing education programs.

Compliant and Safe Operations:

  • Operate company vehicles in compliance with safety and operational standards.

Perform other duties as assigned.

Qualifications

Education and Experience Requirements

  • Current Paramedic certification required; Community Paramedicine certification preferred.
  • Minimum of 3 years of experience as a paramedic, with at least 1 year in a community or mobile integrated healthcare setting preferred.
  • Experience in chronic disease management, patient education, and case management.
  • Proficiency with electronic medical records and Microsoft Office Suite.

Knowledge, Skills, and Abilities

  • State-specific Paramedic certification (required).
  • Advanced Cardiac Life Support (ACLS) certification (required).
  • Pediatric Advanced Life Support (PALS) certification (preferred).
  • Basic Trauma Life Support (BTLS) or Pre-Hospital Trauma Life Support (PHTLS) (preferred).
  • Valid drivers license with an insurable driving record.
