About You:

• Doctor of Dental Surgery (DDS) or Doctor of Medicine in Dentistry (DMD) degree from an accredited dental school, or currently attending an accredited dental school to obtain a degree for DDS or DMD.
• Current, valid dental license to practice in the state or the ability to obtain.
• DEA certification
• CPR/BLS certification or the ability to obtain
