SCCMHA JOB VACANCY ANNOUNCEMENT

CLASSIFICATION: Care Coordinator - Health Services (Medical Assistant / Community Health Worker)

PAY GRADE: $20.36 - $23.68 hourly

POSITION SUMMARY:

Under the administrative supervision of the Supervisor of Business Practice the Health Home Specialist (HHS) and the clinical supervision of the Chief of Health Services and Integrated Care, supports consumers and health professionals by performing routine tasks and procedures as part of a team approach for providing consumer care. The HHS will function as a health navigator by serving as the first point of contact and respond appropriately to internal or external providers seeking to arrange or clarify consumer care by ensuring that callers receive a same day or next workday response This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.

ESSENTIAL DUTIES AND RESPONSIBLITIES:

1. Answer telephones, schedule appointments, perform updating or scanning of consumer medical records.

2. Collect biometrics and rooms consumers.

3. Completes documentation in real time.

4. Supports the appointment registration process for the co-located primary care provider (Great Lakes Bay Health Centers) and laboratory drawing services (Quest Diagnostics).

5. Provide self-screening tools to consumers, as directed, who are receiving services in the Health Home. Screening tools will be completed by consumers and returned to HHS. Consumer responses will be recorded in SENTRI in real time. Assist consumers who may need help in reading or understanding questions.

6. Enhance health home services by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

7. Maintain accurate, real time, documentation in Sentri.

8. Prioritizes responding to staff and consumer inquiries for assistance in navigating a wide array of health care services.

9. Work with Health Home team members in a support role with the ability to prioritize and change job duties on short notice to fulfill the needs of the department.

10. Provides information and direction to all consumers, visitors, and the general public in a prompt, courteous and professional manner at all times.

11. Always maintain consumer confidentiality.

12. Utilizes Supervisor for guidance and resolution of issues.

13. Maintain current knowledgebase of SCCMHA offices, department heads, specific services available and locations of other community service agencies.

14. Gives directions to consumers/visitors coming into the building.

15. Assist consumers in coordinating transportation to and from appointments.

16. Assists consumers in navigating the Community Mental Health System, including service array, consumer rights, and the appeals and grievance procedures.

17. Performs secretarial/receptionist duties for the function of maintaining an effective Health Home department.

18. Performs other duties as assigned by Supervisor.

19. Work in conjunction with RN and respond to (clinic action request) per policy guidelines.

20. Explain the process of medication review, assessments, or evaluations to consumers.

21. Maintain clinic equipment and supplies.

22. Adheres to the mission, vision, core values and operating principles of SCCMHA at all times.

INCIDENTAL DUTIES AND RESPONSIBILITES:

1. Attend and participate in Health Home and Integrated Care staff meetings as scheduled.

2. Works in conjunction with security customer services department acting as point person for consumers entering SCCMHA arriving for appointments with the health home.

3. Attends meetings, in-services training, seminars, conferences, and workshops in order to sustain professional competency, as approved by supervisor.

4. Able to work with children and adults with serious mental health diagnoses.

5. Other duties as assigned.

REPORTING RELATIONSHIPS:

Reports to: Administrative supervision: Supervisor of Business Practice-Health Services. Clinical supervision: Chief of Health Services and Integrated Care.

WORKING CONDITIONS/ENVIRONMENT:

Works in an office environment with usual pressures of time constraints and stress from long periods of computer use. Must be available to move around all facility locations on an as needed basis. May encounter individuals dealing with symptoms related to mental health diagnosis. These symptoms may present as irritability, agitation, and aggression. Drives vehicle in all kinds of weather.

QUALIFICATIONS:

Education: High School diploma or equivalent with courses in general business, typing, bookkeeping, office practices, and information technology. Coursework in healthcare preferred.

Experience: Three (3) years’ experience as a secretary or receptionist, working in social work field, healthcare area, or in a related field.se Experience in the use of multi-line phone system (minimum 5 external lines and 10 internal extensions). This minimum requirement may be waived at SCCMHA discretion based upon the candidate’s overall job experience.

Lived experience with mental illness and/or substance use disorder is beneficial.

Licenses and Certifications: Valid Michigan Driver’s license with a good driving record.

Medical Assistant (MA) training and/or certification, Community Health Worker (CHW) training and/or certification, Peer support Training and/or certification, or Peer Recovery Coach training and/or certification, is preferred.

Knowledge, Skills, and Abilities:

1. Knowledge and support for person centered planning concepts.

2. Demonstrates the ability to work effectively with individuals with mental illness, individuals with developmental disabilities, and children, and families.

3. Ability to use tact and diplomacy and understand individuals from all social, economic, and cultural backgrounds.

4. Ability to plan, organize and complete work in a timely fashion.

5. Effective oral and written communication skills.

6. Ability to work as a member of a team.

7. Ability to accept constructive supervision.

Physical/Mental Requirements:

1. Hearing acuity to converse in person and on telephone.

2. Visual acuity to observe consumers behavior, read and proofread documents and use EHR and other electronic devices.

3. Ability to walk, stand, or sit for extended periods of time.

4. Able to operate standard office equipment (PC, Keyboard, Copy Machine, Fax Machine, etc.)

5. Ability to regularly lift and carry files and supplies at least 20 pounds. Ability to reach, bend, or stoop to manage charts or supplies up to 20 pounds, must also have the ability to push or pull large records cart.

6. Strong interpersonal skills to interact with leadership, employees, consumers, and the general public.

7. Mental capacity to think independently, follow instruction and use judgment.

8. Ability to plan short and long range and to manage and schedule time.

9. Ability to handle stress in meeting deadlines and dealing with large numbers of employees and/or consumers.
