What You'll Do

• Provide one-on-one home-based physical therapy

• Complete evaluations and build individualized treatment plans

• Educate patients and caregivers on mobility and safety

• Collaborate with nursing and therapy partners

• Travel within the Salem service area for home visits

Qualifications

• Active PT license or eligibility for Oregon licensure

• Doctorate or Master's in PT

• 1+ year of PT experience (home health preferred)

• CPR/BLS certification

• Reliable transportation
