- Lead, coordinate, and oversee all direct and indirect patient care services.
- Supervise and mentor clinical staff to ensure exceptional quality and adherence to organizational standards.
- Conduct regular chart audits to ensure compliance and maintain quality assurance.
- Conduct performance appraisals, coaching, and ongoing professional development for nursing and aide staff.
- Lead and participate in QAPI activities; gather and review quality data.
- Review intake information and determine appropriateness for hospice services.
- Ensure compliance with Medicare, state, and CHAP regulations.
- Maintain accurate, complete patient clinical records.
- Manage clinical supply inventory.
- Support onboarding and orientation of new clinical staff.
- Provide patient visits in the home setting as needed.
- Lead interdisciplinary team (IDT) meetings to drive patient-centered, holistic care.
- Maintain ongoing professional growth through education, conferences, and industry involvement.
- Graduate of an accredited School of Nursing
- Registered Nurse (RN) licensed in the state of Arizona
- Minimum 2 years of hospice or home health nursing experience (hospice strongly preferred)
- Strong understanding of regulatory standards (Medicare, state, CHAP)
- Excellent communication skillswritten, verbal, and interpersonal
- Confident, positive leadership style with a passion for team development
- Ability to collaborate, motivate, and build strong relationships
- Strong organizational and problem-solving abilities
