Overview:

Care Management Clinician Behavioral Health Care Management

The Care Management Clinician plays a vital role in supporting youth and families engaged in the NCH Behavioral Health Intensive Care Management (ICM) program. This position is designed for a highly collaborative, clinically informed professional who is committed to improving outcomes for children and adolescents experiencing significant mental health challenges.

Valid Ohio drivers license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Childrens Hospital and must maintain qualification of insurance guidelines. Provides case management procedures, therapeutic interventions and clinical care coordinationfor children, adolescents and their families.

Job Description:

Essential Functions:

  • Conducts assessments of patients' mental health needs and develops individualized care plans. Monitors patients' progress and adjusts care plans as needed.
  • Coordinates client care with other service providers and collaborates with interdisciplinary team to ensure continuity of care.
  • Coordinates available NCH and community resources to enhance client care.
  • Provides education to patients, families, and care teams about mental health conditions and treatment options.
  • Performs brief therapeutic interventions and counseling.
  • Completes clinical charting and other related administrative paperwork in accordance with policy standards.
  • Participates in quality improvement initiatives to improve patient outcomes.

Education Requirement:

Master's degree in Counseling, Social Work or Marriage and Family Therapy, required.

Licensure Requirement:

  • Appropriate Ohio licensure, required.Must remain current in required licensure and clinical or other training as required.
  • Independent license or satisfactory progress toward independent licensure, required.

Excellent communication and interpersonal skills in order to establish and maintain positive relationships with clients, families, peers and community representatives.

Experience:

Experienceproviding and managing behavioral healthcare to children, adolescent and families, required.
